The office of personal management, that's OPM, uses optional form 178 or OF 178 to document the results of that exam. Despite the paperwork reduction Act of 1980 and despite many doctors finding it surprising that all but the most unusual people have eight pages of body parts that can be examined without surgery, the OF 178 is eight pages long. The first page consists only of instructions for which all of us may be grateful. They point out that the form has five sections, identified remarkably as Parts A, B, C, D, and E. So far, so good. Those are not as uninteresting as they may seem, as we're about to learn. Part A on page 2 is to be completed by the new employee and requires him or her to know what has been written in Part B on page 3. How the information in Part B came to be on page 3 instead of page 2 is a mystery and is the subject of a forthcoming children's bedtime story. In what might be a Eureka moment for physicians, the instructions in Part A require the prospective employee to "explain fully to the physician" any "error-interfering medical disorder or physical impairment."
